The Elizabeth Hospice Clinical Team Supervisor supervises the delivery of patient/family care across the life span (pediatrics and adults). Applies expert clinical judgment to ensure optimum quality of patient care and services and assists the Team Manager(s) as directed. Assists with Quality Assessment and Performance Improvement Program by working on projects related to patient care as assigned.

The Elizabeth Hospice is San Diego's oldest non-profit hospice of 40 years. We are growing and looking for talented individuals to join our high performing team and support our mission. We serve all of San Diego county and south Riverside county with multiple hospice office locations.

Our Mission: To enhance the quality of life for those nearing the end of life's journey and for those who grieve.

Work Schedule: Monday through Friday, 8:00 a.m.-5:00 p.m.

Salary: $100,000-$115,000 annually. This is a salaried, exempt position.

Full Time Position 80 hours per pay period.

Travel required between Carlsbad, Escondido, and Temecula offices and supervision for patient visits.

Occasional travel for educational purposes.

On-call commitment: Required.

Responsibilities
    Complies with all federal, state, and local health care regulations as they affect the provision of clinical services.
  1. Reviews weekly/monthly reports that show budget variances and works with manager to identify cost saving methods.
  2. Assists hiring managers with the hiring process by interviewing and assessing candidates and providing recommendations on hiring decisions. Directly supervises and evaluates the clinical practice and performance of LVNs and Hospice Aides. This includes and is not limited to training, performing Field Observation Visits, scheduling and assignments.
  3. Participates and offers recommendations in plan of care decisions and keeps informed of treatment and management plans for patients and their caregivers.
  4. Co-facilitates Interdisciplinary Group Meetings with clinical team manager.
  5. Facilitates optimum client satisfaction by coordinating team activities.
  6. Supports same day admission and registration visits in collaboration with the Access Center leadership.
  7. Completes same day admissions when required as assigned.
  8. Assigns newly admitted patients.
  9. Supports the planning, coordination, and delivery of patient/family care in the absence of other Team Managers.
  10. Collaborates with Clinical Team Manager, Compliance and Quality Assurance Departments to investigate and resolve quality concerns.
  11. Participates in agency on call procedures.
  12. Collaborates with interdepartmental managers to assess deficiencies and determine process improvement needed.
  13. Responsible for project development, implementation, and evaluation as directed.
Qualifications
  1. Bachelor's Degree preferred, preferably in nursing, health care administration, or business administration, and a minimum of 2 years in an increasingly responsible supervisory position, or equivalent combination of education and experience.
  2. Requires knowledge of hospice care, the dying and bereavement process and demonstrated clinical expertise in managing patient and family concerns.
  3. Current CA RN licensure required.
  4. Certification in hospice and palliative care preferred.
  5. Current CPR certification.
  6. Current negative TB skin test.
  7. Proficient in electronic documentation and basic computer skills (Word processing, Data Entry, Outlook, Word)
  8. Current California driver's license with proof of auto insurance required.
